Our care

Care that grows with your loved one.

From early-stage assisted living through advanced memory care and end-of-life support, residents stay in the same home with the same trusted team — even as their needs change.

Assisted Living

Personal support, private dignity.

For seniors who want to live in a loving home with help available exactly when needed — and out of the way when it isn't.

  • Personal care: bathing, dressing, grooming
  • Medication management directed by registered nurses
  • Mobility assistance and fall prevention
  • Three home-cooked meals from scratch, daily
  • Laundry, housekeeping, and linen service
Dementia & Memory Care

A calm, familiar place.

For residents living with dementia, the environment matters as much as the clinical support. APCH homes are sized and staffed to feel like home — quiet, warm, and predictable.

  • Trained dementia care across every shift
  • Structured daily routines that reduce anxiety
  • Memory-friendly home design with secure outdoor space
  • Music, sensory, and reminiscence programming
  • Family education and ongoing communication
Alzheimer's Care

Through every stage.

Alzheimer's is a long road, and the care needed at year one looks very different from year five. Our residents stay in the same home, with the same team, as their needs evolve — no transfer to a new building, no new caregivers to meet.

  • Stage-appropriate care plans, reviewed regularly
  • Specialized communication techniques
  • Behavioral support without unnecessary medication
  • End-of-life care in place, when the time comes
Parkinson's Care

Specialists who understand.

Parkinson's care requires precise medication timing, careful mobility support, and a team that recognizes what an "on" day vs an "off" day looks like. Our caregiving team has years of specialized experience.

  • Precise medication timing protocols
  • Mobility and balance support
  • Speech, swallowing, and nutrition awareness
  • Coordination with neurology and movement disorder specialists
Hospice & End-of-Life

Home, until the end.

One of the things that matters most to families is knowing their loved one won't have to move at the end of life. APCH partners with respected local hospice providers so residents can receive end-of-life care in the home they've come to know.

  • In-home hospice partnership coordination
  • Family room and overnight accommodations for loved ones
  • Bereavement and family support resources
  • Comfort-focused care, in the home they call theirs
Visiting Physician

Medical care, your way.

A visiting physician is available to APCH residents who choose her services — meaning routine medical care can happen in the home, without a stressful trip to a clinic. Residents who prefer to keep their own physician are always welcome to do so.

  • Optional visiting physician for residents who choose her care
  • Coordination with the resident's existing doctors and specialists
  • Medication review and prescription coordination
  • RN direction available to all homes, 24 hours a day
Personal Services

Looking and feeling good.

Manicures, pedicures, haircuts, and massage therapy are available at every APCH home. At our Luxury Living homes — The Chateau and The Castillo — these services are complimentary. At our other homes, families coordinate directly with the vendors of their choice.

  • Manicures & pedicures
  • Haircuts & styling
  • Massage therapy
  • Complimentary at Luxury Living homes
